About the Event

Youth Luv n Prep is a one-day youth convening hosted by Louisiana Just Recovery Network (LJRN/HG) bringing together young people ages 14–18 from Southeast Louisiana’s frontline communities. The event takes place at the LJRN/HG warehouse and includes hands-on disaster preparedness skill stations (including outdoor and physical activities such as roof tarping practice and non-perishable food preparation), youth panels, and a closing ceremony with certificate of completion.

The event takes place primarily inside and around the LJRN/HG warehouse space, which is a working facility. Youth will participate in supervised activity stations throughout the day. LJRN/HG staff and our member org staff will be present at all times. LJRN members who are participating and bringing their youth groups include: Zion Traveller’s Cooperative, Familias Unidas en Accion, Rural Roots LA, Louisiana Organization for Refugees and Immigrants, and Bayou Youth Leadership Project.

Description of Activities and Known Risks

Activities planned for this event include but may not be limited to:

• Indoor and outdoor preparedness skill stations, which may include roof tarping practice (on a training platform 2 ft off ground), disaster kit assembly, and emergency food handling

• Mental health wellness activities led by facilitators

• Youth panel and group discussion

• Physical movement and hands-on activities in a warehouse setting

I understand that participation in these activities involves some inherent risks, including but not limited to: physical activity, use of tools and materials, outdoor exposure, and movement in a warehouse environment. I understand that LJRN/HG has taken reasonable precautions to ensure participant safety and that staff and facilitators will be present throughout the event.

Parental Consent and Permission

I, the undersigned parent or legal guardian, hereby grant permission for my child named below to participate in Youth Luv n Prep hosted by Louisiana Just Recovery Network/Healthy Gulf on July 8, 2026. I understand that this is a one-day event with no overnight component.

I consent on behalf of my child to:

• Full participation in all scheduled programming, activities, and skill stations

• Participation in photography or video recording, as described in the Media Release section below

• Emergency medical care, if LJRN/HG staff or designees deem it necessary

Release of Liability – Parent / Guardian

To the fullest extent permitted by applicable Louisiana law, I, as parent or legal guardian, hereby release, discharge, and agree to hold harmless Louisiana Just Recovery Network, Healthy Gulf (LJRN/HG’s fiscal sponsor), and their respective directors, officers, staff, employees, volunteers, and agents (collectively “Released Parties”) from any and all claims, demands, damages, costs, expenses, actions, and causes of action arising out of or related to my child’s participation in Youth Luv n Prep or presence at the LJRN/HG warehouse facility on July 8, 2026.

This release includes claims arising from the ordinary negligence of the Released Parties. I understand and agree that this release does not cover gross negligence, recklessness, or intentional misconduct.

I further agree that LJRN/HG has no responsibility to provide medical care to my child and makes no offer to do so, but I authorize LJRN/HG staff, should they deem it necessary, to call emergency services, administer basic first aid, or arrange transport to a medical facility on behalf of my child. I agree to be financially responsible for any costs related to medical care or emergency services incurred on my child’s behalf.

Supervision and Conduct

I understand that:

• LJRN/HG staff and facilitators will provide general supervision of all youth participants during programming.

• Each participating member organization is responsible for accompanying and their youth to and from the event and for ensuring appropriate adult-to-youth supervision ratios during transit.  Each participating member org is also responsible for supervision of youth during the event.  

• Youth participants are expected to follow instructions from LJRN/HG staff and facilitators and to conduct themselves in a manner consistent with the values of the event: respect, curiosity, and community care.

By signing below, I certify that: (1) I am the parent or legal guardian of the youth participant named above; (2) I have read and fully understand the terms of this form; (3) I voluntarily consent to my child’s participation in Youth Luv n Prep; and (4) I agree to the terms of the release, medical authorization, and media consent as checked above.

If signing electronically, I agree that my electronic signature is the legally binding equivalent of my handwritten signature on paper.
